Heartsbane is a two-handed Valyrian Steel sword wielded by Randyll Tarly. You can find the sword from ACOK 2.1 onwards.

You can get Heartsbane by killing Randyll Tarly (or waiting for him to be killed) and then picking up his weapon during a battle.
